CHF Receives $10,000 Grant from the Robert Glenn Rapp Foundation to Support Caregiver Training for Medically Fragile Children

OKLAHOMA CITY – Children’s Health Foundation (CHF) is pleased to announce a $10,000 grant from the Robert Glenn Rapp Foundation to support the From Hospital to Home Caregiver Training Project, a program that equips families with the knowledge and confidence needed to care for medically fragile children transitioning from hospital care to home.
This training program focuses on families whose children rely on tracheostomies, ventilators, feeding support, or other complex medical equipment. Structured, hands-on instruction helps caregivers manage daily needs, troubleshoot equipment, and respond to emergencies. The program reduces preventable hospital readmissions, strengthens family readiness, and ensures safer, more stable home environments for medically vulnerable children.
“We are incredibly grateful to the Robert Glenn Rapp Foundation for investing in this lifesaving work,” said Brandon Brooks, Chief Executive Officer of Children’s Health Foundation. “Their support helps ensure that caregivers feel prepared and supported as they bring their child home, and it directly improves outcomes for some of the most medically fragile children in our state.”
